The liver is the body's principal metabolic and detoxification organ, performing several hundred distinct functions: converting nutrients into usable fuel, storing glycogen and fat-soluble vitamins, synthesizing albumin and clotting factors, producing bile for fat digestion, and transforming drugs, alcohol, hormones, and environmental compounds into forms the body can excrete. It possesses an unusual capacity for regeneration, but that resilience also masks damage — significant injury often produces no symptoms until function is substantially compromised. Common conditions include metabolic dysfunction-associated steatotic liver disease, alcohol-related liver disease, viral hepatitis, drug-induced injury, and the fibrosis and cirrhosis that can follow any sustained insult.
Liver biology divides into recognizable processes. Detoxification proceeds in phases: cytochrome P450 enzymes oxidize substrates in phase I, conjugation by glutathione S-transferases, UDP-glucuronosyltransferases, and sulfotransferases follows in phase II, and transporters export the products in phase III. Lipid handling involves de novo lipogenesis via SREBP-1c, fatty acid oxidation via PPAR-alpha, and VLDL export. When these are overwhelmed, oxidative stress, mitochondrial dysfunction, endoplasmic reticulum stress, and Kupffer cell activation with NF-kB signaling drive inflammation, and hepatic stellate cells activate through TGF-beta to deposit collagen.
