The liver is the body's principal metabolic and detoxification organ, performing several hundred distinct functions: converting nutrients into usable fuel, storing glycogen and fat-soluble vitamins, synthesizing albumin and clotting factors, producing bile for fat digestion, and transforming drugs, alcohol, hormones, and environmental compounds into forms the body can excrete. It possesses an unusual capacity for regeneration, but that resilience also masks damage — significant injury often produces no symptoms until function is substantially compromised.
Detoxification proceeds in phases: cytochrome P450 enzymes oxidize substrates in phase I, conjugation by glutathione S-transferases, UDP-glucuronosyltransferases, and sulfotransferases follows in phase II, and transporters export the products in phase III. When these systems are overwhelmed, oxidative stress, endotoxin accumulation, and innate immune activation with NF-κB signaling drive injury.
