Muscle gain, or hypertrophy, is the enlargement of skeletal muscle fibers that occurs when the rate of muscle protein synthesis exceeds the rate of breakdown over a sustained period. It is driven principally by mechanical loading — resistance training that imposes high tension on the fiber — combined with adequate protein and energy intake, sufficient recovery, and a supportive hormonal environment. Adaptation is not limited to fiber size: neural efficiency improves early, connective tissue remodels, capillary density increases, and satellite cells donate additional nuclei to support a larger cytoplasmic volume. Beyond athletic performance, maintaining muscle mass is central to metabolic health, glucose disposal, bone loading, injury resilience, and preserving function with age.

At the molecular level, mechanical tension is transduced through integrins and focal adhesion complexes, converging on mTORC1, the central regulator of translation initiation via S6K1 and 4E-BP1. Leucine availability and insulin-like growth factor signaling reinforce this pathway. Opposing it, the ubiquitin-proteasome system acting through MuRF1 and atrogin-1, along with autophagy and myostatin signaling via Smad2/3, drives protein degradation. Satellite cell activation, mitochondrial biogenesis, androgen receptor signaling, and inflammatory tone modulate the net result.
