Bowel health describes how well the large and small intestine perform their combined roles: completing digestion, absorbing water, electrolytes, and nutrients, housing the majority of the body's microbial population, and maintaining a selective barrier between gut contents and the bloodstream. When these functions falter, symptoms range from the everyday — bloating, irregularity, cramping, urgency — to defined disorders such as irritable bowel syndrome, inflammatory bowel disease, diverticular disease, and small intestinal bacterial overgrowth. Because roughly seventy percent of immune tissue lines the gut, intestinal condition also shapes systemic immunity, and because the gut synthesizes and responds to neurotransmitters, it influences mood and stress responses as well.

Several biological processes determine bowel function. Barrier integrity depends on tight junction proteins such as occludin and the claudins, on mucin production by goblet cells, and on secretory IgA. Microbial composition governs fermentation of dietary fiber into short-chain fatty acids like butyrate, the preferred fuel of colonocytes and a regulator of regulatory T cells. Motility is coordinated by the enteric nervous system and interstitial cells of Cajal. Dysbiosis, mucosal inflammation via NF-kB and inflammasome activation, and bile acid signaling complete the picture. This page decomposes each process into pathways and biomarkers.
